Investigations Into Deadly BP Refinery Explosion Uncover Repeated Problems

British Petroleum (BP) North America Inc decided not to wait for the natural end to a series of investigations into a deadly explosion at one of its five refineries in March, concluding soon afterwards that “management failures and employee mistakes” were to blame for the catastrophe. The Ergonomics Report reviewed several of the interim investigations in November and talked to experts to see if the blame is misplaced.
